At ProActive Long-Term Care Homes of Sudbury, we truly believe in a ProActive approach to every step by implementing the care model framework. Our team conducts an extensive intake process to determine the needs of the individual, including but not limited to: assistive devices for activities of daily living, life skills programs, meeting with physiotherapists to develop a physical rehabilitation plan, occupational therapists, speech language pathologist, ect. Our team will ensure that the client is always safe, well cared for to the highest possible standard of care, while building a strong therapeutic rapport to ensure that we always maintain truth, compassion and dignity to all.

We offer a wide range of services to meet the unique needs of each individual, such as

- Specific peer support groups

- Group fitness activities that are inclusive to all levels of mobility

- Respite care services

- Attendant care services

- Physical rehabilitation programs "supervised by physiotherapists".

- Behaviour management services in conjunction with Registered Health Care Professionals.

- Medication management, which includes inspecting blister packs from the pharmacy to ensure that the medication always follow the five "R's" of medication administration such as 

1. Right Medication

2. Right Dose

3. Right Person

4. Right Time

5. Right Delivery Method

At ProActive we hold our namesake to heart and ensure that our clients are properly assessed by a Registered Health Care Professional such as Occupational Therapists to ensure that each client has a tailor made program to suit their individual needs before work even begins with a client to ensure safety, comfort and reliability to our clients. Should a client need rails for walking, a ceiling lift for transferring in or out of bed or assistive devices such as specially tailored plates and cutlery, we at ProActive believe in setting up our clients for success so that they can live their lives to the fullest and not let any physical limitation hold them back.

We understand that finding the right people and investing in our staff to ensure that they have the most up to date training in personal care and attendant services while also ensuring that our staff are properly compensated for their hard work. We at ProActive do not pretend that this field is easy, however that does not mean that working in this field must be difficult or seem like you are walking up a greased ramp. At ProActive we ensure that all staff have three consecutive days off every week to allow them to rest properly and maintain a healthy work-life balance. Our employees are paid appropriately so that they do not need to work a second job to support themselves. At ProActive we believe that a happy and healthy work force is the key to fully achieving success for the clients.

Here at ProActive we recognize that dealing with medication and pharmacies can be difficult which is why we offer our medication management program. We work hard to ensure that our clients have a family doctor that can support them and their needs, we also believe in going the extra mile with medication safety by helping our clients to better understand their medications and how those meds work for them. we also provide Blister Pack Checks where our staff will ensure that all medications are thoroughly counted and are accurate by the provided description from the pharmacy.
